Wasilla Lions Club Vision Screening Project
Through a collaborative effort of the Wasilla Lions Club, Ophthalmic Associated and Mat-Su School District Health Services, we will be offering PlusopticX Vision Screening. This vision scanner takes an instant “radar” reading of your child’s eyes, which is then interpreted by computer. Conditions that can be detected are listed on the permission slip under “Reasons for Vision Screening”.
- There is no charge to participate
- The parent permission form must be filled out and signed
- The results of the screening will be sent home by the school nurse.
The information obtained from the screening is to be considered a preliminary procedure only, and does not constitute a diagnosis of vision problems. It should be part of a comprehensive eye care program, which includes periodic ophthalmic exams.
Parents are responsible for arranging a full eye exam with an eye care professional, if the child has been referred as a result of screening.
The organization conducting the screening will not be held accountable for any errors of commission, omission, or other misdiagnosis.
If you wish for your child to be a part of this screening project, complete the Parent Permission form and return the form to the school. Your child will not be screened without this form being completed. if you have any questions, please call me at the nurse’s office at 352-7459
